Informal Assessments in Transition Planning helps determine students' transition needs. Informal assessments that encompass nine domains are included. They are: employment, further education/training, self-determination, daily living, communication, health, interpersonal relationships, leisure activities, and community participation.

This is one of my favorites. The resource is user friendly and affordable. I observed one teacher modify the "Individual Supports Assessment Form" for non readers with picture software to assist with gathering information about goals, preferences and personal experiences. In addition, I utilized this resource to gather information in a variety of the domain areas overtime to develop transition plans and future's plan (Person Centered Plan). The resources are adaptable for all ability levels.
At the school I currently work at we were given this informal assessment book to reproduce and use with our students and their family. I like this book a lot. I use the parent survival checklist most often and I send this home for the parent's to fill out. I usually include the results in the IEP's Present Level of Performance. This assessment helps to give the parent a voice in the IEP; it provides the parent voice about what they feel their child needs more assistance with. Such as, cooking, cleaning, managing money etc. I use other assessments out of this book as well and typically send this assessments home to be completed. In my opinion it's worth the money.
I have used several assessments from this book. It is very useful to me as I have a wide variety of abilities with my class. This allows me to use the appropriate level. There are several that are extremely quick and allow a quick glance. I would recommend this tool for its price and ease of use. If you need more in depth material, this will allow you an opportunity to decide what areas you look into. For the price, it is well worth the investment.

I have used several assessments in this book. They are very helpful for many reasons. One, is that they are quick and easy to use and administer. They have a variety of different kinds that focus on different areas and needs. And most importantly, they have assessments for different ages - which i find extremely useful considering the need to use "age appropriate" assessments. I also use the assessments as tools to enhance self-advocacy and interview skills.
I have used these rating scales in my Transitions class. The students found them user friendly and a basis for good communication of their experiences and needs.
I have not used this book, but trying to better understand the book is difficult. The website doesn't give you enough to make an educated decision just a brief explaination which is unfortunate. If this book is worthy of being used in our schools resource inventory publishers should allow prospective buyers to sample the books assessments.
I would like to be able to see more of the assessment. I like the domains covered. Based on other teachers review from our school, I am interested in trying it.
I really like what this assessments suggest that it includes. It appears that it covers a wide range of areas that are beneficial in the transition process and could be useful in helping students determine their strengths, interests and goals. However it was not helpful that I could not view sample pages of the assessment to make a true opinion of whether ot not I think this assessment would be helpful to the transition team.
