Transition Assessment Guidance

Need some guidance? The importance of transition assessment during transition planning should not be understated!

As educators, we spend time each day assessing students for the purposes of tracking progress, targeting areas for improvement, and providing feedback. However, we may not consider the value and significance of any type of systematic transition assessment. In addition, we are required to identify post-secondary goals based upon age-appropriate transition assessments related to training, education, employment, and, where appropriate, independent living skills. Therefore, it is even more important for special educators to know about and use effective transition assessments.

How is your program doing on:

  • Providing a wide range of formal and informal assessments to students.
  • Assessments that evaluate based on the individual student.
  • Implementing ongoing transition processes.

What are your outcomes with:

  • Meaningful sharing of results with staff, students, and family.
  • Student postsecondary goals  based on transition assessment results.
  • Summary of performance and recommendations for postsecondary goals development when students finish high school.

Do you struggle with:

Developing a transition assessment plan?

Identifying appropriate assessments?

Interpreting and applying data?

Understanding how to use effective assessments?

These critical components are at the core of  meeting  individualized student needs.
